Terms of Service (Public Website & SMS Communication)

These Terms of Service ("Terms") govern use of the Addiction & Recovery Research Institute, LLC (“ARRI,” “we,” “us,” or “our”) website, intake forms, and related electronic communications, including SMS/text messaging. By submitting an intake form, contacting us, or engaging in services, you acknowledge and agree to these Terms.

1. Services Provided

ARRI provides behavioral health and substance use disorder assessment, treatment, and recovery support services. Submission of an intake form does not establish a provider–patient relationship. Services begin only after clinical intake, informed consent, and eligibility determination.

2. Electronic Communications & SMS Consent

By submitting an intake or contact form, you consent to receive service-related communications from ARRI via phone call, email, and SMS/text message. These communications may include appointment scheduling, reminders, intake follow-up, care coordination, and administrative notices.

Message frequency varies based on engagement and care needs.

Message and data rates may apply depending on your mobile carrier and plan.

You may opt out of SMS messages at any time by replying STOP. You may reply HELP for assistance.

3. Privacy & Confidentiality

ARRI is committed to protecting your privacy. Use and disclosure of personal and health information are governed by our Privacy Policy and applicable federal and state laws, including HIPAA and 42 CFR Part 2. We do not sell or share personal information for marketing or promotional purposes. Information may be shared only with authorized staff, subcontractors, or service providers as necessary to deliver services or comply with legal obligations.

4. No Emergency or Crisis Services

Electronic communications and intake forms are not monitored for emergencies. If you are experiencing a medical or psychiatric emergency, call 911 or your local emergency number immediately.

5. Accuracy of Information

You agree to provide accurate, current, and complete information when submitting forms or communicating with ARRI. Providing false or misleading information may delay or prevent services.

6. Website & Form Use

You agree not to misuse ARRI’s website or forms, attempt unauthorized access, transmit harmful code, or use the site for unlawful purposes.

7. Limitation of Liability

ARRI is not liable for delays, interruptions, or failures in electronic communications beyond our reasonable control, including carrier issues or technical outages.

8. Changes to These Terms

ARRI may update these Terms periodically. Updates will be posted on this page with a revised effective date.

I understand that my substance use disorder records are protected under federal law, including the federal regulations governing the confidentiality of substance use disorder patient records, 42 C.F.R. Part 2, and the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), 45 C.F.R. Parts 160 and 164, and cannot be disclosed without my written consent unless otherwise provided for by the regulations.
